How to refer patients in South West London
1. Send the completed referral form to ovivauk.t3wm@nhs.net. Please complete all sections and ensure that clinical measurements are obtained within date reference.
2. Alternatively, find our referral form on Arden’s Local document library (in areas that commission the service), via SystmOne or EMIS). This will pre-populate much of the information that we need.

Who does your team consist of?
We are consultant-led and have a multidisciplinary team to support our patients. This team includes Consultant endocrinologists, specialist GPs, nurses, dietitians, psychologists, psychology wellbeing practitioners, health psychologists and health coaches.

Our programme combines medication with tailored diet, exercise, and behavioural guidance to help patients build lasting healthy habits. This wraparound care reduces the risk of weight regain and ensures patients maintain their results beyond treatment.
