How to refer patients in Leicester, Leicestershire and Rutland
1. Access the Referral Form
- EMIS users – Import the auto-populating referral form template using one of the following methods:
a) Import using Resource Publisher (see instructions here)
b) Import using Template Manager (see instructions here)

2. Submit the Referral
Once completed, please send the referral form and any relevant attachments using one of the following methods.

On our Tier 3 programme at 12 months, patients achieve an average 15% weight loss, with 80% app retention at 6 months, alongside a 45% reduction in sick days and 43% fewer GP visits – demonstrating the impact of our wrap-around, specialist support.
Who does your team consist of?
We are consultant-led and have a multidisciplinary team to support our patients. This team includes Consultant endocrinologists, specialist GPs, nurses, dietitians, psychologists, psychology wellbeing practitioners, health psychologists and health coaches.
Will my patient put the weight back on once they have stopped taking the weight loss medications?
Our programme combines medication with tailored diet, exercise, and behavioural guidance to help patients build lasting healthy habits. This wraparound care reduces the risk of weight regain and ensures patients maintain their results beyond treatment.
