About Early Onset Type 2 Diabetes Support
This 12-week programme will help you to better manage your type 2 diabetes, improve your blood glucose levels, lose weight if you need to and build new habits to lead a healthier, happier lifestyle. The programme is free of charge and accessible via smartphone app, or over the telephone.
Who can join?
To participate in the programme, you must be:
- Aged between 18 – 39
- Registered in an area where Oviva is available
- Have a confirmed diagnosis of type 2 diabetes

How is the programme delivered?
The programme is delivered by a company called Oviva, who work in partnership with the NHS, via smartphone app or over the telephone. There will be no need to travel for appointments – you can take part from the comfort of your own home, at a time that suits you.
What happens on Oviva Early Onset Type 2 Diabetes Support?
You will be matched with a specialist Health Coach who will work with you to develop your own personalised healthcare plan. Over a period of 12 weeks, they will support you to make sustainable lifestyle changes and help you to learn more about type 2 diabetes. Backed by the latest science, you will receive new resources each week to support your journey towards a healthy body and mind. Through the Oviva app, you will also be able to track your food and activity, and to set and monitor goals to help you achieve the results you want.

To get started quicker, you have the option to provide extra information (e.g., your latest HbA1c reading). Find this information by using the NHS app or by contacting your GP practice.
