When patients join, we will assess their needs and understand their preferences to provide them with their own personalised healthcare plan. They will have the choice to be matched with either their own personal health coach or a coach-led support group to help them throughout their journey. They will also have the choice to receive support either via the Oviva app over secure app messaging, or over phone/video calls. Throughout the journey, they will have support at regular intervals with a specialist weight management dietitian. We will assess their need for psychology support and they may also receive support from a psychologist or psychology practitioner.
Patients will be able to use the Oviva app to track their progress, log their food, mood, activity and communicate with their coach. Throughout the 12 month programme, new learning modules and resources on Oviva Learn will be available for patients to read, watch and listen. This will provide them with information on how to best manage their dietary approach, lifestyle changes, diabetes and weight management, to help them with making long-term changes to their lifestyle.

At 3 months average weight loss was 6.1kg (4.5%) for the app group vs. 5.3kg (4.1%) for the face to face group. At 6 months average weight loss was 9.2kg (6.8%) for the app group vs. 7.6kg (5.6%) for the face to face group.

Suitability for weight loss medication (SaxendaⓇ) is reviewed by the multidisciplinary team as part of their medical assessment at week 12 of the programme. The Oviva team has the ability to prescribe weight loss medication (SaxendaⓇ) if appropriate, and patients are supported with this by a Diabetes Specialist Nurse. If patients are eligible for weight loss medication, the pathway extends to 24 months.

The programme is not appropriate for people with a significant learning disability or untreated mental health condition when it prevents them from engaging with the programme. However, if a patient has a carer, friend or family member who can support them throughout the programme and you feel is able to engage and understand the learning materials then please forward the referral to us. We would assess each referral on a case by case basis. Please provide as much detail as possible in the referral to assist with this. Oviva Way to Wellness provides specialist multidisciplinary care and participants may have an initial consultation (IC) with a psychology practitioner (depending on triage of needs). We will assess their need for psychology support and they will receive support from a psychologist or psychology practitioner if needed.
-
During the initial assessment we will assess a participant’s need for psychological support. If their triage indicates they require psychological support, they will have an initial consultation (IC) with a psychologist or psychology practitioner. Following this we can offer up to 5 psychology coaching sessions, depending on their level of need.
