About this programme

Healthier You: NHS Diabetes Prevention Programme is part of a national programme. The free digital remote programme provided by Oviva offers personalised support and advice over the phone or via our app.

In this 9 month programme, participants will be supported to feel more confident managing their health, improve their blood glucose levels, lose weight if they need to and to build new habits to support leading a healthier, happier lifestyle.

How to refer

Why should you refer your patients?

The 9 month programme empowers participants to achieve and maintain healthy habits through long term behaviour change. The programme aims to help develop and maintain healthier habits in all aspects of health and wellbeing. This includes guidance on nutrition and diet, physical activity, stress management, sleep, mindfulness and much more. The programme also aims to build confidence in managing health long after the programme has ended.

Patients can choose to take part in the programme digitally using our online resources and app, or can access an offline programme if they don’t have a smartphone or would prefer phone calls. Patients are in control of how they’d like to take part so they can interact with the programme in the best way for them.

Programme pathway

Oviva’s provision of the NHS Diabetes Prevention Programme offers a variety of pathways that are tailored to our participants needs. This flexible approach allows participants to chose a coaching method that suits their lifestyle and motivational requirements. Participants will chose from one to one or group communication with their health coach via secure messaging in the Oviva app or by telephone calls. 

Start

After completing an initial assessment, we’ll use participant information to triage and assign the most suitable pathway. A health coach will then be in touch to offer their tailored support via in app messaging (one to one or in a group) or via a telephone call. 

Change

The first 12 weeks focus on intensive behaviour change to work towards personal health goals with the support from a health coach or their peers. This helps in the early stages of making long term lifestyle changes.

Sustain

For the remainder of the programme participants will continue to build their confidence in maintaining new healthier behaviour long term, with monthly support.

Participants will continue to self-track activities and grow their knowledge using the weekly online resources, to support long term changes. 

Eligibility criteria

  • People aged 18 or over
  • People who are not pregnant
  • No confirmed diagnosis of Type 2 diabetes
  • Those who are motivated to change their lifestyle and can commit to the 9 month programme
  • Registered in an area where Oviva is available

How to refer your patients

How can primary care refer to the programme?

After confirming eligibility and discussing the service with your patient please complete the referral form which is embedded into your clinical system. 

What resources are available for patients who do not want to or are unable to use the Oviva app?

If a participant does not have a smartphone or would prefer not to use one, they can have all appointments via telephone calls. The individual is in control of how they would like to take part and so can participate with the programme in the best way for them. The app is not compulsory and you can complete the full Oviva Diabetes Support programme without using the app if preferred, although we strongly encourage all participants to download the app to have the best possible experience of the programme.
