One to One Programme provided by Oviva

About this programme

The Transform Diabetes Study is offering people from selected GP practices in Southwark the opportunity to follow a choice of diets that could achieve a significant improvement in blood sugar levels or diabetes remission. All participants will be supported for one year by a Diabetes Specialist Dietitian.

How does it work?

We ask that participants are able to commit to a year-long digital behaviour change programme with the aim of losing weight, improving diabetes control and in some cases, putting diabetes into remission.

Working with your dietitian, you’ll understand how to safely complete your chosen dietary intervention over 12 weeks to achieve weight loss and manage your medications too. For the remainder of the programme, you will work on sustaining new healthy habits to keep your weight loss and improvements to your diabetes management.

The provision of Tier 3 weight management services varies across the UK, and there are multiple barriers to attendance.

As part of your plan, you will be paired with your own personal Diabetes Specialist Dietitian to support you in making changes as you progress through the programme.

Use the Oviva app to track your weight, food, activity, mood, blood glucose and blood pressure. New resources in Oviva Learn are unlocked frequently. The videos, podcasts, recipes, quizzes are all written by our specialist dietitians and psychologists, and will support you throughout every stage of the programme to help you work towards your health goals.

The three dietary interventions that you can choose from are:

Total Diet Replacement (TDR)

This is a diet whereby you consume four meal replacement products per day for 12 weeks in place of food. Food is then slowly reintroduced and you are coached to form new, healthy habits to sustain your weight loss in the long term. If you choose to follow the TDR, all products will be funded for you.

Low-carbohydrate diet

Carbohydrate breaks down into sugar and raises blood sugar levels, and high blood sugar levels are associated with diabetes. Lowering carbohydrate intake, by reducing intake of foods such as bread, potatoes, rice, pasta and sugary foods, can improve blood sugar levels as well as offering weight loss.

5:2 diet

The 5:2 diet is an example of intermittent fasting. When following this diet, your calorie intake is significantly reduced on two days of the week, to 500 calories per day for women and 600 calories for men, whilst eating healthily on the other five days of the week.

Over the first period of 12 weeks I lost about 2.5 stone. If you can get through the first hard few weeks (like with anything!) – the benefits are great, it really did help. I didn’t have the urge to eat more which was a concern I had that I’d still be hungry. Now I’ve seen the benefits I don’t want to go back to how I was feeling before.
